![]() Referral Process We accept referrals from many different sources throughout the county, including social services, community support agencies, therapists, schools and families. If you are interested in referring a child to our program; please complete the PPP referrals application. Upon receipt of the application by PPP staff, the Program Director will review it for approval. Once the application has been approved, the Intake Coordinator will be in contact with you to begin the authorization process. If you are interested in the child receiving educational services while attending day treatment with PPP, please complete the ALP Application, in addition to the Referral Application In order for a child to participate in our day treatment program, Community Support Services must be in place. If the child does not presently have community support, his/her guardian should contact our intake coordinator to request information for accessing community support services and for general information about admittance to our program. Once Community Support Services are in place, the Intake Coordinator will work closely with the community support provider to complete the authorization process.
