Characteristics of Puzzle Piece Programs Consumers

Child is under the age of 21
Child may have been under supervision for a mental health disorder
Child may have been short or long term suspended or expelled from school
Child has emotional or mental health problems, behavioral and/or social issues
Child's mental and/or emotional symptoms are delaying developmental progress and/or not allowing them to fulfill age appropriate responsiblities Child may have attempted or threatened suicide


Symptomatic Indications of Clients Needing PPP

Young/inexperienced parent(s) or teen pregnancy
Little or no support system i.e. family and friends
Low income
Frequent moves
Changes in primary caregivers
Lack of education
Death of someone close
Substance abuse/alcoholism
Family stressors
Physical/verbal Aggression


Typical Symptoms Associated with PPP Clients

Poor school grades/Low academic achievement
Problems with attitude i.e. Defiant or disrespectful towards those in authority
Lying
Stealing
Drinking/Smoking/Drugs
Aggression
Anger
Bad language/Profanity
Sexual activity/Promiscuity
Skipping school/Running away
Court/DJJ Involvement
Gang Involved
Anxiety
Destructive
Trouble Maintaining Friendships


Referral Resources

Referrals are received from many different sources throughout the county, including:
Community Support Providers
Social Services/Child Protective Services (CPS)
Foster Care Provider Agencies
Court Counselors
Managed Care Agencies
Therapists
Schools
Residential Facilities/PRTF's/Group Homes/Hospitals
Families


Puzzle Piece Programs is happy to accept referrals from any source. However, consumers are required to have Community Support Services in place prior to enrolment in to the program. The Intake Coordinator will be available to assist with this process.
